Silver Cross in New Lenox to help residents with health care Power of Attorney documents

]Silver Cross Hospital is offering free assistance with completing health care power of attorney documents in honor of National Health Care Decision Day on April 16.

Health care professionals will be available to answer questions about advanced care planning and assist with completing the Illinois Health Care Power of Attorney form.

This free program will be held from 10 a.m. to 1 p.m. April 16 in the Silver Cross Hospital Sangmeister Library, 1900 Silver Cross Blvd., New Lenox. The library is located on the hospital’s first floor, just off the main lobby.

Health care power of attorney documents are recommended for all persons 18 years and older.

Appointing an agent for health care is important to ensure that a person’s health care decisions are made in alignment with their treatment preferences or wishes. Patients can make their own decisions, but the decisions need to be in writing.
